Role overview
This position is for a Clinical Pharmacist to support general practice teams across multiple sites with prescription handling, medicines management, and safer prescribing. The role focuses on repeat prescribing, urgent prescription requests, medicines reconciliation when patients move between care settings, and expert clinical guidance on medicines for the benefit of both patients and the wider care team. The post is designed to improve the quality, efficiency, and cost-effectiveness of clinical services within K&W Healthcare and its member practices.
Key duties
The pharmacist will carry out clinical medication reviews, including more complex cases that require longer appointments, to help reduce pressure on GPs and locum doctors. The role includes checking ongoing medicine needs, supporting correct use of medicines, managing minor ailment triage, and running patient-facing clinics to improve adherence and answer medication-related questions.
Additional work includes reconciling medicines after hospital discharge, reviewing daily pathology results for selected patients, and completing focused reviews for higher-risk groups such as frail older adults, patients on multiple medicines, and those with renal or hepatic impairment or high-risk medicines. The post holder will also provide prescribing recommendations to nurses and GPs, administer influenza and travel vaccinations, support patients with alcohol or drug dependence, and direct them to suitable services where needed.
The role also involves participation in multidisciplinary medication reviews, audits related to medicines linked with unplanned admissions, and actions that help maintain continuity of supply for vulnerable groups, including care-home residents. The pharmacist will contribute to QOF delivery and long-term condition management for asthma, COPD, diabetes, hypertension, and heart failure, while also improving repeat prescribing systems through liaison with dispensing community pharmacies, prescription authorisation, and practice policy development.
Training and development
The post holder is expected to complete training and certification in health assessments, history taking, and physical examination. An independent prescribing course should be started within 6 months of probation. The role also includes helping practices implement drug withdrawals, safety alerts, and NICE medication guidance, while using computer searches to identify high-risk patient groups.
Qualifications and compliance
Applicants must be registered with the General Pharmaceutical Council and hold current UK professional registration. Previous experience in general practice and independent prescribing are desirable. This role is subject to a Disclosure and Barring Service check under the Rehabilitation of Offenders Act (Exceptions Order) 1975.
Work locations
The role is based across multiple London-area sites, including 22 Fryent Way, Kingsbury, London, NW9 9SB; 116 Chaplin Road, Wembley, Middlesex, HA0 4UZ; and Gladstone Medical Centre, 5 Dollis Hill Lane, London, NW2 6JH.
