Role overview

Senior Doc is hiring an experienced Care Management Leader in Anaheim, CA to guide and grow a large care operations team of about 50 Care Managers and Medical Assistants supporting a Chronic Care Management (CCM) program at scale. The position reports to the Chief Operating Officer and centers on day-to-day leadership, workflow improvement, performance tracking, and building a team culture grounded in accountability and strong service for patients and providers.

This opportunity is best suited to someone who has led complex, metrics-oriented operations and enjoys shaping high-performing teams in a fast-moving healthcare environment.

What you will do

  • Direct and support a team of roughly 50 Care Managers and Medical Assistants.
  • Hire, onboard, coach, mentor, and continuously develop team members.
  • Create a workplace culture focused on ownership, teamwork, speed, and ongoing improvement.
  • Hold regular coaching discussions, performance evaluations, and career growth planning conversations.
  • Develop and maintain dashboards, scorecards, and reports covering productivity, CCM minutes, documentation quality, compliance, and overall operational results.
  • Run daily team huddles to align on volume, quality, and workload priorities.
  • Track performance trends, clear obstacles that slow the team down, and improve efficiency through better processes.
  • Design scalable workflows and operating procedures that improve consistency and outcomes.
  • Review productivity data and key metrics to spot improvement opportunities.
  • Celebrate top performers and provide targeted coaching to team members who need additional support.
  • Use performance improvement plans and corrective action when needed.
  • Share operational findings and strategic recommendations with senior leadership.
  • Keep CCM documentation aligned with payer rules and compliance standards.
  • Support audit readiness across all care management activities.
  • Balance productivity goals with strong patient and provider experience.
  • Act as the escalation contact for complex patient or provider issues.
  • Work closely with clinical, operations, technology, and administrative teams to improve care delivery.
  • Help assess and implement new technology, workflow changes, and process improvements that strengthen efficiency and patient outcomes.

What the company is looking for

  • A bachelor's degree in Healthcare Administration, Business Administration, Nursing, or a similar discipline.
  • At least 7 years of progressive leadership experience with large, high-volume, metrics-led teams.
  • Minimum 3 years of experience leading people managers or sizable operational teams.
  • Hands-on experience creating dashboards, KPIs, scorecards, and performance reports.
  • Strong analytical ability and a track record of turning data into practical operational improvements.
  • Experience working with EHR systems, care management platforms, and reporting tools.
  • Excellent leadership, communication, coaching, and organizational abilities.
  • A master's degree such as an MBA, MHA, MSN, or an equivalent advanced degree is preferred.
  • Background in Chronic Care Management, Population Health, Value-Based Care, or Care Management is preferred.
  • Clinical licensure such as RN, LCSW, or a similar credential is an advantage.
  • Experience leading healthcare operations, patient engagement, customer service, enrollment, call center, hospitality, retail operations, or other high-volume environments is beneficial.
