Role Overview
Senior Doc is seeking an accomplished Care Management Leader to guide and grow a team of roughly 50 Care Managers and Medical Assistants supporting a large Chronic Care Management (CCM) program. In this position, you will report to the Chief Operating Officer and take ownership of day-to-day operations, process improvement, performance tracking, and team culture. The role is centered on building accountability, improving workflows, and delivering excellent experiences for both patients and providers.

Leadership and Team Development
- Oversee a workforce of about 50 Care Managers and Medical Assistants.
- Hire, orient, coach, mentor, and support team growth.
- Create a work environment grounded in accountability, teamwork, urgency, and ongoing improvement.
- Hold regular coaching conversations, performance evaluations, and development planning discussions.
Operational Excellence
- Design and manage dashboards, scorecards, and reports that track care manager productivity, CCM minutes, documentation quality, compliance, and overall operational results.
- Run daily huddles focused on output, quality, and workload balancing.
- Track team performance and remove roadblocks that slow execution.
- Build scalable workflows and processes that improve consistency and business outcomes.
Performance Management and Data Analysis
- Review productivity patterns and operational data to uncover opportunities for improvement.
- Acknowledge strong performers and provide additional coaching to employees who need support.
- Put performance improvement plans and corrective actions in place when necessary.
- Share operational findings and strategic recommendations with senior leadership.
Quality, Compliance, and Escalations
- Keep CCM documentation aligned with payer requirements and internal standards.
- Ensure operations remain audit-ready at all times.
- Balance efficiency goals with strong patient and provider satisfaction.
- Act as the escalation contact for complex patient or provider issues.
Cross-Functional Collaboration
- Work closely with clinical, operations, technology, and administrative partners to enhance care delivery.
- Assess and introduce new tools, workflows, and process enhancements that support efficiency and better patient outcomes.

Compensation
The posted salary range for this region is USD 140,000 to USD 167,000 per year.
