Overview
Senior Doc is seeking a seasoned Care Management Leader to guide and grow a team of about 50 Care Managers and Medical Assistants supporting a large Chronic Care Management (CCM) program. The role reports to the Chief Operating Officer and is centered on daily operational leadership, process improvement, team development, and maintaining strong patient and provider experiences.
This position is well suited for an operations professional who excels in a fast-moving, metrics-focused setting and enjoys building motivated, high-performing teams.
Key Responsibilities
In this role, you will lead team performance, strengthen workflows, and use data to drive operational results. You will also partner across departments to improve care delivery and support compliance standards.
- Oversee roughly 50 Care Managers and Medical Assistants, providing day-to-day leadership and direction.
- Hire, welcome, coach, mentor, and support the ongoing growth of team members.
- Promote a workplace culture built on accountability, teamwork, urgency, and continuous improvement.
- Hold regular coaching discussions, performance evaluations, and career planning conversations.
- Create and maintain dashboards, scorecards, and reporting tools to track productivity, CCM minutes, documentation quality, compliance, and overall performance.
- Run daily huddles focused on productivity, quality, and workload distribution.
- Track team output and remove barriers that affect efficiency.
- Design scalable processes and workflows that improve consistency and results.
- Study productivity patterns and operational data to identify improvement opportunities.
- Recognize strong performers and support employees who need additional guidance.
- Implement performance improvement plans and corrective action when needed.
- Share operational findings and strategic recommendations with senior leadership.
- Ensure CCM documentation and payer guideline compliance are consistently met.
- Keep operations audit-ready across the care management function.
- Balance productivity goals with excellent patient and provider satisfaction.
- Act as the escalation contact for complex patient or provider issues.
- Work closely with clinical, operations, technology, and administrative partners to improve care delivery.
- Assess and adopt new technologies, workflows, and process enhancements that improve efficiency and patient outcomes.
Requirements
- Bachelor’s degree in Healthcare Administration, Business Administration, Nursing, or a related discipline.
- At least 7 years of progressive leadership experience leading high-volume, metrics-driven teams.
- Minimum 3 years of experience managing people managers or large operational groups.
- Hands-on experience building dashboards, KPIs, scorecards, and performance reports.
- Strong analytical ability with the capacity to turn data into practical operational improvements.
- Experience with EHR platforms, care management tools, and reporting systems.
- Excellent skills in leadership, communication, coaching, and organization.
- Preferred: Master’s degree such as an MBA, MHA, MSN, or a similar qualification.
- Preferred: Background in Chronic Care Management, Population Health, Value-Based Care, or broader care management operations.
- Preferred: Clinical licensure such as RN or LCSW, or an equivalent credential.
- Preferred: Experience in healthcare operations, care management, patient engagement, customer service, enrollment, call centers, hospitality, retail operations, or other high-volume environments.
