Position Overview

We are looking to hire a qualified Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our home care team. The selected professional will provide high-quality nursing care to patients in their homes, collaborating closely with families and other healthcare providers to promote the best possible patient outcomes.

Key Responsibilities

Evaluate patients' health status and create personalized care plans
Administer prescribed medications, injections, and treatments
Monitor vital signs and keep accurate records of patient progress
Provide wound care, catheter maintenance, and assorted medical procedures
Educate patients and their families on managing diseases and sustaining health
Coordinate care efforts with physicians, therapists, and other health professionals
Ensure documentation complies with regulations and is timely
Respond promptly to emergencies with appropriate interventions
Travel to various patient homes within the service area

Requirements

Valid RN or LPN license for Delhi NCR region
CPR/BLS certification
Minimum of 2 years nursing experience; home care experience is highly desirable
Strong clinical assessment abilities and critical thinking skills
Excellent interpersonal and communication skills
Ability to work independently and efficiently manage time
Compassionate and patient-focused care approach

Preferred Qualifications

Prior experience in home health or community nursing
Familiarity with electronic medical records (EMR) systems
Specialized training in geriatrics, chronic disease management, or wound care
Multilingual abilities (if applicable)
Possession of a valid driver’s license and reliable transport

Physical and Other Requirements

Ability to lift up to 50 pounds
Capacity to stand, walk, and bend for prolonged periods
Willingness and ability to travel to multiple patient locations daily within a fixed zone
