Position Overview

We are looking to hire a qualified Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our home care team. The selected professional will provide high-quality nursing care to patients in their homes, collaborating closely with families and other healthcare providers to promote the best possible patient outcomes.

Key Responsibilities

• Evaluate patients' health status and create personalized care plans
• Administer prescribed medications, injections, and treatments
• Monitor vital signs and keep accurate records of patient progress
• Provide wound care, catheter maintenance, and assorted medical procedures
• Educate patients and their families on managing diseases and sustaining health
• Coordinate care efforts with physicians, therapists, and other health professionals
• Ensure documentation complies with regulations and is timely
• Respond promptly to emergencies with appropriate interventions
• Travel to various patient homes within the service area

Requirements

• Valid RN or LPN license for Delhi NCR region
• CPR/BLS certification
• Minimum of 2 years nursing experience; home care experience is highly desirable
• Strong clinical assessment abilities and critical thinking skills
• Excellent interpersonal and communication skills
• Ability to work independently and efficiently manage time
• Compassionate and patient-focused care approach

Preferred Qualifications

• Prior experience in home health or community nursing
• Familiarity with electronic medical records (EMR) systems
• Specialized training in geriatrics, chronic disease management, or wound care
• Multilingual abilities (if applicable)
• Possession of a valid driver’s license and reliable transport

Physical and Other Requirements

• Ability to lift up to 50 pounds
• Capacity to stand, walk, and bend for prolonged periods
• Willingness and ability to travel to multiple patient locations daily within a fixed zone
