Position Overview

The Care Coordinator plays an essential role in linking individuals needing long-term care with benefits and resources offered through the Washington Cares Fund (WA Cares) initiative, designed to provide Washington workers access to long-term care benefits. This position is responsible for conducting client assessments and assisting eligible individuals with benefits navigation under the WA Cares program. The Coordinator will collaborate closely with the Intake & Eligibility Coordinator, Program Manager, and other staff to ensure smooth access to long-term care benefits and support program implementation.

This is a full-time, non-exempt union role based in Seattle with hybrid working arrangements. Initial employment requires onsite presence Monday through Friday, 8:30 AM to 5 PM, for the introductory 90 days. Post successful completion of this period, the role may transition to a hybrid schedule requiring onsite or field presence Tuesday through Thursday. Field visits will be limited to King County, with responsibilities at Lifelong's Georgetown Yards and Capitol Hill offices.

Key Responsibilities

  • Conduct benefits assessments for WA Cares beneficiaries, collecting required documents and beneficiary data, evaluating needs, communicating determinations, and documenting outcomes.
  • Assist clients in benefits navigation, including researching providers, negotiating rates, and utilizing long-term care benefits effectively.
  • Coordinate and support the intake and eligibility processes collaborating with the WA Cares team.
  • Manage logistical, organizational, and administrative tasks related to WA Cares applications, ensuring assessments are processed promptly.
  • Provide compassionate, knowledgeable customer service reflective of WA Cares Fund policies and culture.
  • Maintain professional communications with clients, internal teams, and community partners through various channels.
  • Utilize a client-centered outreach approach through phone calls, meetings, mailings, and partnerships with care providers to ensure continuous enrollment and service access.
  • Perform accurate and high-volume data entry, including client profile management, document scanning, report generation, database maintenance, and mail merges.
  • Carry out assessments to confirm if individuals meet functional eligibility criteria per WA Cares Fund protocols, updating data in the GetCare system.
  • Collaborate with Aging & Disability Services leadership to implement and improve program policies and procedures.
  • Establish and maintain cooperative relationships with internal departments and external care partners to support client transitions and ongoing care continuity.
  • Participate actively in team meetings, trainings, and organizational events to build knowledge and support organizational culture.
  • Maintain respectful interactions and uphold confidentiality while working reliably during established hours.
  • Engage in Lifelong’s Employee Performance Management Process.

Qualifications and Core Competencies

  • Master’s degree in behavioral or health sciences with one year of paid social service experience; or a Bachelor’s degree in these fields plus two years’ experience; or a Bachelor’s degree with four years of social service experience.
  • Proficiency with Microsoft Office Suite, especially Excel; familiarity with Access, Word, PowerPoint, and Outlook.
  • Extensive experience with high-accuracy data entry and electronic document management.
  • Strong organizational and multitasking skills with minimal supervision.
  • Dedication to equity, inclusion, and client-centered service across diverse populations.
  • Excellent verbal and written communication, customer service, and phone etiquette.
  • Ability to work independently and collaboratively within a team.
  • Adaptable with ability to maintain composure and self-direction in dynamic environments.
  • Bilingual abilities are preferred but not required.

Compensation and Benefits

  • Hourly wage range: $27.60 to $42.63

Work Environment and Additional Information

  • Employees working directly with clients must undergo a tuberculosis test within 30 days of hire and annually thereafter as required.
  • Employment offers are contingent on successfully passing criminal background checks; some programs may require more frequent or additional checks.
  • The job primarily involves office work with extended computer use and desk sitting, occasional standing, walking, and light lifting up to 20 pounds, with potential attendance at events outside normal hours or settings.
