Job Overview

The Care Coordinator plays a vital role in ensuring uninterrupted patient care by organizing medical services and addressing the needs of patients effectively.

Key Responsibilities

  • Engage proactively with clients to promote wellness and preventive health measures, including encouraging regular medical examinations, screenings, vaccinations, and healthy lifestyle habits.
  • Work alongside healthcare providers to create individualized care plans focused on wellness and disease prevention.
  • Conduct health risk assessments and screenings to detect potential health concerns and advise on preventive strategies.
  • Manage screening processes for conditions such as hypertension, diabetes, cancers, and infectious diseases, based on individual or population health needs.
  • Distribute information and resources related to wellness initiatives and programs.
  • Arrange referrals to specialists and preventive health services tailored to client requirements.
  • Facilitate clear communication among healthcare professionals to ensure optimal patient care and follow-up.
  • Act as the primary liaison connecting clients, healthcare providers, and other medical personnel.
  • Support efficient documentation and transitions in care through effective communication.
  • Monitor patient adherence to preventive care plans, including scheduled screenings and recommended interventions.
  • Follow up with clients to overcome any obstacles, provide encouragement, and emphasize the importance of preventive care.
  • Establish and nurture partnerships with community organizations to broaden access to preventive resources and programs.
  • Maintain detailed and accurate records of client interactions, care plans, and outcomes.
  • Prepare necessary reports in compliance with healthcare regulations to assess the impact of wellness and preventive efforts.
  • Adapt to working across multiple locations as required.
  • Perform additional duties to support wellness objectives.

  • Uphold person-centered care principles in daily duties.
  • Participate actively in continuous improvement projects within the job scope.
  • Adhere strictly to safety protocols and proactively resolve safety issues.
  • Ensure confidentiality of sensitive information encountered during work.
  • Comply with cybersecurity policies and engage in necessary training to safeguard health systems.
  • Maintain high ethical standards and comply with organizational codes of conduct.
  • Complete required mandatory training at least one month before expiration.

Required Qualifications and Skills

  • Solid knowledge of preventive care principles, covering screenings, vaccinations, lifestyle choices, and health promotion.
  • Updated on contemporary guidelines and best practices in wellness and preventive health services.
  • Excellent interpersonal and communication skills for engaging clients, healthcare workers, and key stakeholders.
  • Ability to simplify complex medical concepts for diverse client groups and establish strong rapport.
  • Experience in care planning and coordination, including assessment, development of personalized care plans, and managing multi-provider services.
  • Understanding of healthcare system navigation and the importance of care continuity.
  • Capability to motivate and educate clients about preventive health.
  • Strong organizational and time-management abilities to handle caseloads and follow-up efficiently.
  • Ability to manage multiple responsibilities within established guidelines.
  • Competence in data collection, analysis, and interpretation to evaluate wellness program outcomes and improve care delivery.
  • Cultural sensitivity and awareness of diverse patient backgrounds to provide equitable, client-focused care.
  • Proficiency with electronic health records, care coordination software, and related healthcare technology platforms.
  • Good understanding of health information privacy standards.
  • Effective collaboration within interdisciplinary teams, working well with healthcare professionals and community partners to advance wellness.

Experience and Education

  • Minimum of three years' experience as a clinical coordinator or a comparable role within a healthcare setting.
  • Bachelor's degree in healthcare-related fields such as nursing, public health, health education, or healthcare administration.
  • Additional certifications in care coordination, client engagement, or wellness programming are advantageous.

Languages and Licenses

  • Excellent proficiency in spoken and written English; Arabic proficiency is preferred.
  • Valid registration and classification from the Saudi Commission for Health Specialties are required.
