Interactive Community Care (ICCare), founded in 2010, specializes in the Community Services sector and has served NSW communities for over ten years. This multidisciplinary care organization provides out-of-home care, crisis support, and disability services across various regions in NSW, addressing the complex care requirements of children, adolescents, and adults.
Role Overview
The Lumiere Foundation seeks a skilled NDIS Support Coordinator to assist ICCare participants by managing diverse levels of support coordination and transitions within their NDIS plans.
Key Responsibilities
- Follow and uphold NDIS and Child Safe standards and regulations rigorously.
- Coordinate participant supports effectively within allocated funding.
- Collaborate with key stakeholders to align service delivery towards participants' goals.
- Empower participants by facilitating choice and control to access various formal and informal community supports, emphasizing innovative service options.
- Maintain precise and clear records, files, and reports adhering to guidelines, standards, and legislative requirements related to NDIS and Quality and Safeguards.
Qualifications and Experience
- Possess tertiary qualifications in social sciences, health, or a related professional discipline, or a diploma in Disability Services studies.
- Have a minimum of two years’ practical experience in Support Coordination.
- Familiarity with NDIS Quality and Safeguards Commission practice standards.
- Proficiency with Microsoft Office Suite (Outlook, Word, Excel) and experience utilizing CMS systems such as MYP, Lumary, or ShiftCare.
- Arabic language skills are advantageous but not mandatory.
- Strong verbal and written communication skills in English, including report writing capabilities.
Compliance Requirements
- Must have unrestricted right to live and work in Australia.
- Hold a current National Police Check.
- Possess a valid First Aid Certificate.
- Maintain a satisfactory Working with Children Check and NDIS Worker Screening Check.
- Have a full unrestricted NSW driver’s license and access to a reliable, roadworthy vehicle.
