About the Role
We are hiring a Clinical Pharmacist to play an integral role in enterprise-wide clinical initiatives, supporting provider and patient engagement, optimizing medication use, enhancing clinical decision support, and contributing to drug strategy projects. Collaborating closely with medical leaders, care teams, and operational partners, you will work to elevate care quality, patient outcomes, and cost efficiency within a value-driven care setting.
Key Responsibilities
- Participate in pharmacist-led clinical programs focusing on chronic conditions, quality enhancements, and medication optimization.
- Proactively engage eligible patients through outreach, enrollment, education, and follow-ups to maintain continuous engagement.
- Perform Medication Reconciliation, Medication Therapy Management (MTM), and Comprehensive Medication Reviews (CMR).
- Deliver telephonic education to patients about medication usage, adherence, self-care techniques, and when to escalate care.
- Offer actionable medication recommendations collaborating with providers via technology and direct communication to support practice adoption and medication management.
- Provide consultation on pharmacy-related issues across other clinical programs.
- Promote medication adherence to address gaps in quality measures.
- Work collaboratively within the interdisciplinary team including primary care physicians, health coaches, and nursing staff to create patient-specific care plans and reconcile medications.
- Facilitate adoption of clinical insights by providers through promoting the use of actionable drug recommendations.
- Contribute feedback to improve clinical decision support tools’ logic, usability, and effectiveness.
- Partner with Regional Medical Officers, Market Medical Directors, Practice Transformation Managers, care teams, and practices to ensure uniform clinical standards and workflow consistency across regions.
- Support initiatives related to specialty medications, high-cost drugs, and pharmacy optimization aligned with value-based care.
- Ensure all clinical activities are documented accurately following state and federal regulations, including HIPAA, using designated platforms and technology.
- Serve as an information resource concerning pharmacy practices for providers and internal clinical teams.
- Maintain current knowledge of clinical guidelines, quality metrics, and pharmacy best practices.
Candidate Profile
- Possess a Doctor of Pharmacy (PharmD) degree.
- Hold an active pharmacist license in good standing from states including CA, WA, KS, OH, NY, NJ, FL, MD, NH, MS, or LA.
- UMPJE certification preferred.
- Experience in Medication Therapy Management and pharmacist-led clinical programs.
- Strong knowledge of evidence-based clinical guidelines and quality standards.
- Interest or prior experience in value-based care, population health, or Accountable Care Organization (ACO) environments.
- Exemplify strong clinical judgment with capacity to make independent medication recommendations.
- Capable of working across multiple clinical programs, utilizing data analytics tools and coordinating with operational teams.
- Adept at working with partial or evolving clinical information.
- Ability to drive clinical program implementation and achieve measurable results.
- Excellent interpersonal and communication skills when engaging providers, patients, and stakeholders.
- Organized, adaptable, and effective in dynamic and fast-evolving settings.
- High standards of professionalism, integrity, and accountability.
