Role Overview

Based in Alexandra Hospital, Singapore, the Social Work Coordinator plays a vital role in supporting patients through financial aid facilitation and discharge planning, while assisting Medical Social Workers with case management and departmental administration.

Key Responsibilities

• Manage administrative and operational tasks within the Social Work department, including overseeing service counters.
• Engage directly with patients and their families via intake interviews, financial assessments, and coordinating necessary support and referrals for discharge planning in select care areas.
• Assist Medical Social Workers by providing translation services, patient escorting, and aiding home visit arrangements.
• Contribute to work process enhancements and participate in quality improvement initiatives.
• Collaborate with hospital departments and external organizations to improve patient care experiences.
• Support departmental programs and projects as needed.

Qualifications and Requirements

• Must possess at least a Diploma in Social Work.
• Good interpersonal communication and problem-solving abilities are essential.
• Proficiency in IT skills relevant to the role is required.
• The candidate will report to a Medical Social Worker and work closely with the Social Work Coordinators Team Leader.
• Applicants must be Singapore Citizens or Permanent Residents.
