Job Overview
Join a multidisciplinary healthcare team focused on delivering comprehensive and compassionate care. This role empowers patients and their caregivers to proactively manage their health via technology, improving access to and awareness of community health resources.
Key Objectives and Responsibilities
- Establish and maintain productive relationships with both internal care teams and external stakeholders including patients and their caregivers.
- Support clinical goals by promoting preventive health screenings, reducing hospital readmissions and escalation to advanced care facilities, and collecting relevant social determinants of health to positively impact patient outcomes.
- Utilize technology tools to enhance patient health literacy and facilitate patient access to healthcare resources.
- Coordinate and assist with patient care activities, ensuring thorough documentation and tracking of clinical care indicators using IT systems.
- Perform basic health parameter assessments; close preventive screening and immunization gaps aligned with national health programs and initiatives.
- Deliver fundamental health education to foster better lifestyle habits, encourage physical and social activities through social prescribing, and advise on the use of health monitoring devices and vaccination schedules.
Qualifications and Requirements
- Diploma qualification is preferred; a background in health promotion is advantageous.
- Prior experience in healthcare settings is desirable.
- Strong communication skills, both verbal and written.
- Possess a pleasant demeanor and good interpersonal skills; must demonstrate a positive work attitude and the capability to collaborate as part of a team.
- Effective communication and organizational skills are essential.
- Self-driven with genuine interest in patient care.
- Proficiency in English and at least one local language or dialect is required.
- Skilled in Microsoft Office suite including Excel, Word, and PowerPoint.
- Comfortable navigating various IT programs and systems.
