About the Role

The Senior Director, Operations - Evergreen Provider Group & Patient Engagement will own the overall financial and operational performance of attributed patients and the Patient Engagement Services team. This leadership position carries dual responsibility: improving Evergreen Provider Group outcomes while ensuring the engagement team successfully reaches patients, drives scheduling, and supports care completion.

This is a hands-on strategic leadership role for someone who has managed P&L performance, scaled virtual care models, and led end-to-end patient funnel performance across outreach, scheduling, and visit completion. Success in the role depends on strong analytical discipline, performance management, and cross-functional influence in a value-based care setting.

Key Responsibilities

Evergreen Provider Group operations and strategy

  • Direct performance for Evergreen-aligned patients across all markets and payer types.
  • Work closely with the Chief Clinical Officer to expand EPG into a scalable virtual care delivery model, including the systems, workflows, and team structure needed for growth.
  • Create and execute standardized quality programs that improve performance and outcomes across markets.
  • Own the EGA profit and loss statement.
  • Build and maintain dashboards and reports to monitor KPIs, identify gaps, and drive corrective action.
  • Partner with internal and external stakeholders to improve engagement, documentation, quality, and total medical expense.
  • Lead ongoing management of value-based partnerships, including payer issue resolution and contractual compliance.

Patient Engagement Services operations and strategy

  • Guide the PES function so the team consistently reaches, engages, and schedules patients into care.
  • Set productivity standards and KPI goals, and hold the team accountable for capacity use and outreach effectiveness.
  • Remove barriers that limit patient access.
  • Own the PES program budget.
  • Coordinate with clinical leadership to align outreach activity with provider availability and changing capacity.
  • Partner with Marketing and Technology to launch patient engagement campaigns that improve awareness, outreach performance, and scheduling conversion.
  • Continuously refine outreach methods, workflows, and tools so PES can scale alongside EPG growth.

Leadership and collaboration

  • Lead, coach, and develop the EPG and PES teams while building a high-performance culture with clear goals, regular feedback, and growth opportunities.
  • Work across clinical, operations, quality, and technology teams to keep priorities aligned and patient care goals on track.
  • Perform additional related duties as needed.

Required Qualifications

  • At least 8 years of healthcare operations experience with direct responsibility for P&L oversight, financial performance management, and team leadership.
  • Strong working knowledge of value-based care.
  • Background managing a medical group, physician practice, or clinical operations team; experience in a virtual care environment is strongly preferred.
  • Experience building or overseeing patient outreach, engagement, or access operations such as a call center, scheduling, or care coordination is an advantage.
  • Demonstrated success in building teams, scaling operations, and holding staff accountable to productivity and performance goals.
  • Bachelor’s degree or higher in education, quality, public health, business administration, health services research, or a related discipline is preferred.
  • Intermediate proficiency with Microsoft Office Suite, including Outlook and Teams.
  • Ability to work effectively in a mostly remote setup.
  • Home internet must support at least 25 Mbps download and 10 Mbps upload; cable, fiber, or DSL on a hardwired connection is recommended.
  • Evergreen will supply remote employees with telephony tools and equipment as required for the job.
  • A dedicated workspace with no ongoing interruptions is required to protect member PHI/HIPAA information.

Compensation

The annual pay range for this position is $175,000 to $195,000. Final compensation is based on experience, education, role demand, and other job-specific factors. The position is also eligible for an annual bonus.
