Job Summary

This role involves spearheading the rollout of the Fall Prevention Service across Barking and Dagenham, Havering, and Redbridge (BHR) boroughs. The position is a 12-month secondment, ideal for professionals looking to enhance their skills within a dynamic and supportive NHS Foundation Trust environment. A valid driving license and the ability to travel around the BHR area are essential.

Key Responsibilities

  • Collaborate with multidisciplinary teams across various agencies to embed falls prevention strategies effectively.
  • Utilize evidence-based approaches to optimize fall prevention care plans, patient assessments, treatment accessibility, and overall patient experience.
  • Lead the development and execution of a strategic plan to integrate fall prevention into local healthcare services sustainably.
  • Manage a specialist caseload involving complex patients by conducting thorough assessments and designing personalized therapy interventions and discharge plans.
  • Work independently within community settings, performing holistic assessments considering physical, psychological, social, and environmental factors influencing fall risk.
  • Deliver specialized education and advice regarding fall prevention to patients, families, carers, and healthcare professionals.
  • Maintain accurate clinical documentation following Trust policies and contribute to service improvement initiatives including audits and health promotion activities.
  • Ensure adherence to professional standards set by HCPC, CSP, and clinical governance protocols.

Candidate Profile

  • Experienced healthcare professional with a strong background in falls management and healthcare delivery.
  • Proven track record of leading and engaging in change management and quality improvement projects.
  • Knowledgeable about national guidelines related to falls and bone health initiatives.
  • Capable of exercising considerable professional independence and judgement.
  • Strong clinical reasoning and communication skills, able to thrive in a fast-paced and demanding role.

Additional Information

  • This post will start with an extensive induction and training period at the Trust's Goodmayes head office.
  • The role is subject to a probationary period, except for certain internal applicants.
  • Position attracts a High Cost Area Supplement for Outer London areas, which is 15% of the base salary (pro rata £4,870 to £6,137 annually for part-time staff).
