Intake Coordinator

Position Overview

Addus HomeCare invites applications for a full-time Intake Coordinator position based in Clarksville, TN or Lebanon, TN. The role operates Monday through Friday from 8:00 AM to 4:30 PM CST. This critical position focuses on supporting patients and families by coordinating home health services that aid recovery, management of chronic illnesses, and enable safe living at home.

Responsibilities

• Handle patient referrals via phone and direct interactions.
• Communicate clearly about home health services and benefits to patients, their families, and referral sources.
• Help identify other service options if patients are ineligible for home care.
• Organize and book initial nursing assessments and admissions.
• Prepare and uphold admission packets and required documentation.
• Assist in community outreach to strengthen referral partnerships.
• Provide necessary clerical and administrative support to clinical teams.
• Keep patient records accurate and updated promptly.

Qualifications

• Minimum age of 18 years.
• Preferred: At least one year of experience in home health, healthcare, or a comparable sector.
• Strong skills in organization, communication, and providing excellent customer service.
• Capability to prioritize tasks effectively in a dynamic work environment.
• Accuracy and timeliness in completing required documentation.
• Valid driver's license along with proof of auto insurance.
