Care Navigator Level 1

Position Overview

We are looking for a Care Navigator Level 1 to join our team in a fully remote, full-time capacity within the United States. This role offers an opportunity to be the primary contact for patients and caregivers, helping them through their healthcare journey with our care team. Key Duties

• Act as the main point of contact for patients and their caregivers by managing a high volume of inbound and outbound calls.
• Organize and schedule both in-home and virtual care visits accurately and efficiently.
• Provide patient education about program services while setting empathetic and clear expectations for their care process.
• Handle post-visit requirements including coordination of medical equipment, managing pharmacy requests, and documenting clinical information.
• Ensure meticulous documentation of all interactions within internal systems and Electronic Health Records (EHR).
• Work collaboratively with clinical and operations teams to ensure an exceptional patient experience.

Candidate Qualifications

• Minimum of two years of professional experience in a healthcare call center or other patient-facing roles.
• Familiarity and proficiency with Electronic Health Record software such as Epic, Athena, Cerner, or similar platforms.
• Access to a private, HIPAA-compliant work environment with a closed door.
• Reliable high-speed internet connection with at least 20 Mbps download and 5 Mbps upload speeds.
• Excellent active listening abilities and the capacity to manage multiple software systems simultaneously while maintaining a compassionate approach over calls.
• High school diploma or GED is required.
