Role Overview
The Care Navigator is a vital part of the integrated care team, responsible for being the primary point of contact for patients and families once care begins. This position involves supporting families by answering inquiries, providing educational guidance, facilitating next steps, and ensuring a smooth and supportive patient experience.
Key Attributes of the Care Navigator
- Strong people skills and commitment to delivering empathetic, high-quality service that fosters trust and rapport.
- Proactive problem-solving ability to anticipate challenges and improve processes and patient experiences.
- Meticulous attention to detail for managing administrative responsibilities such as scheduling, documentation, and communication follow-ups.
- Comfortable and adept at using technology including Electronic Health Records (EHR) and scheduling systems, alongside phones, texts, and email.
- Capacity to work both collaboratively within a team environment and independently with ownership of responsibilities.
Primary Responsibilities
- Serve as the main contact for ongoing patients, handling calls, messages, and emails with professionalism and empathy.
- Guide patients and families through their care path, managing appointment scheduling, confirmations, and coordination with providers.
- Manage critical administrative duties such as provider orders, screenings, and other tasks essential to care delivery.
- Provide in-office support by maintaining a welcoming environment, assisting patients and providers, handling mail, restocking supplies, and managing office opening and closing duties.
- Ensure strict compliance with privacy regulations including HIPAA and family consent laws to protect patient information.
Working Arrangement
This is a full-time onsite role requiring presence at the Vienna, VA office five days a week.

Compensation
The salary for this position ranges between $23 and $26 per hour.
