Key Responsibilities

  • Engage members to support and develop their healthcare plans by identifying barriers and offering solutions such as medication reminders, appointment scheduling, and referrals to community support resources.
  • Build trusting and collaborative relationships with members and caregivers to facilitate access to resources that empower positive health changes.
  • Participate in team meetings to review progress towards goals and departmental targets outlined in Service Level Agreements.
  • Accurately collect and document member demographics and healthcare information in the Employee Health Record (EHR) system.
  • Handle inbound and outbound communications with members, healthcare providers, and other stakeholders to support organizational objectives.
  • Conduct daily outreach calls to eligible patients, explaining benefits of the Healthmap Kidney Health Management program and enrolling interested members.
  • Respond to inbound member self-referrals for enrollment into the Kidney Health Management program.
  • Engage members with clarity regarding care terms, using prescribed scripts or talking points to professionally address resistance or concerns.
  • Report complaints and suggest corrective or preventive measures, escalating issues following established protocols as necessary.
  • Perform additional duties as assigned.

Qualifications & Experience

  • Associate degree in a relevant discipline or equivalent experience with appropriate certifications considered in lieu of formal education.
  • Minimum of two years of healthcare experience, ideally with individuals having chronic diseases or behavioral health challenges.
  • Preferred experience in managed care, physician offices, or account management.
  • Familiarity with Medicare and Medicaid programs is preferred.
  • Experience in a metrics-driven work environment is advantageous.

Skills & Competencies

  • Strong problem-solving skills with the ability to implement initiatives effectively.
  • Excellent verbal and written communication capabilities.
  • Ability to prioritize and manage multiple tasks simultaneously.
