Role Overview

Key Responsibilities

  • Manage daily operations of ECM, CS, and CHW programs across all served regions.
  • Ensure consistent completion of care visits, active engagement, and other related activities.
  • Monitor performance metrics such as visit completion rates, time to initial visit, and patient engagement.
  • Detect and resolve execution issues within teams promptly.
  • Direct management of CHW Team Managers and Senior Manager of Community Supports, setting clear expectations for productivity and performance.
  • Hold management accountable through coaching, evaluation, and corrective actions as required.
  • Uphold consistent management procedures across geographic locations.
  • Ensure adherence to payor-defined workflows across all teams; identify deviations and initiate remediation.
  • Provide constructive feedback to Payor Operations regarding workflow inefficiencies or challenges.
  • Collaborate on workflow refinements without assuming design ownership.
  • Balance competing priorities and resource limitations by making informed tradeoff decisions.
  • Escalate operational or systemic issues appropriately to senior leadership or related departments.
  • Coordinate cross-functional efforts to maintain alignment without overlapping responsibilities.

Qualifications

  • Proven direct experience running CalAIM programs (ECM or Community Supports) within California.
  • Over 10 years of healthcare operations experience focusing on care delivery or population health.
  • At least 5 years of experience supervising managers or multi-tier teams, preferably in distributed or field environments.
  • Demonstrated responsibility for comprehensive program execution, including performance oversight and operational results.
  • Success in leading consistent execution across multiple regions or geographic areas.
  • Strong knowledge of Medicaid populations, required documentation, and care coordination processes.
  • Capability to manage high variability environments involving multiple plans, evolving requirements, and operational complexities.
  • Skill in making immediate operational decisions amidst competing priorities.
