Role Summary
The Patient Case Manager functions as a liaison managing the referral intake workflow, maintaining consistent communication between departments, and ensuring efficient transitions of patient referrals. This position involves identifying bottlenecks and collaborating cross-functionally to streamline the referral process.
Key Responsibilities
- Manage and review workflow processes for new and current referrals, employing reporting tools to prioritize patient cases effectively.
- Address and resolve issues or deficiencies in care coordination raised by department managers, sales or marketing staff, and referral sources.
- Assess the status of new referrals within 24 to 48 hours, assisting with classification according to referral checklist criteria.
- Daily monitoring of workflow queues to detect and remedy delays, coordinating with intake staff to accelerate referral processing.
- Collaborate with department managers to devise and implement process improvements and workflow tools.
- Perform additional duties as assigned by management.
Required Qualifications
- Education: High school diploma or GED is mandatory. Must hold a current, unrestricted Licensed Practical Nurse (LPN) or Certified Pharmacy Technician license valid in the practicing state.
- Experience: At least three years of experience in pharmacy, healthcare, or clinical environments managing large volumes of complex patient cases, particularly within intake or admissions referral processes.
- Computer Skills: Proficient in Microsoft Office Suite including Word and Excel.
- Competencies: Strong knowledge of referral workflow processes, exceptional verbal and written communication, meticulous attention to detail, effective time management, initiative, prioritization, sound decision making, organizational capabilities, multitasking, critical thinking, problem-solving skills, and dependability.
