Key Responsibilities

You will partner with clients to develop personalized Health Action Plans and conduct monthly home visits to monitor progress and assist with goal achievement. Facilitating communication and coordination among healthcare providers and acting as an advocate are essential. You will also connect clients to community resources, encourage skills development to support recovery and better health outcomes, and help them navigate healthcare systems independently.

Qualifications

The ideal candidate holds a Bachelor's Degree in Human Services, Social Services, Nursing, or a related discipline, and has at least two years of experience in nursing, healthcare, behavioral health, or human services, either paid or volunteer. A valid driver's license, a clean driving record, auto insurance, and successful background and fingerprint checks are required. Bilingual abilities in English and Spanish are preferred but not mandatory.

Compensation and Benefits

The position offers an hourly pay range from $20 to $24, with the possibility of earning an additional incentive-based pay of up to 10% biweekly.
