About Durham Community Health Centre
Durham Community Health Centre (DCHC) is a registered charity dedicated to providing integrated, accessible, and equitable community-based primary care, wellness programs, and health education for residents of Durham facing healthcare barriers. The organization focuses on equity-driven services targeting priority groups such as Indigenous peoples, Black communities, 2SLGBTQI+ individuals, newcomers to Canada, seniors, and those without a primary care provider. DCHC operates with an interprofessional team comprising physicians, nurse practitioners, nurses, counsellors, dietitians, outreach workers, and administrative personnel who collectively deliver client-centered, integrated clinical and wellness support.
Position Overview
The Manager, Integrated Care & Experience is a key member of the leadership team responsible for executing DCHC's integrated care model across various programs and service areas. Reporting to the Vice President of Integrated Care and Experience, this role involves supervising interprofessional teams to ensure delivery of high-quality, coordinated care. The manager drives program integration, performance excellence, and fosters collaboration throughout the organization. The position requires presence primarily in Oshawa and Pickering with rotation across other DCHC sites as needed.
Key Responsibilities
- Lead deployment of strategic plans aligned with the organization’s Theory of Change and priorities.
- Manage interprofessional staff promoting teamwork, accountability, inclusivity, and innovation.
- Engage in budget planning and oversee program outcomes relative to financial and operational goals.
- Champion initiatives for quality improvement and manage organizational change within assigned programs.
- Partner with HR and senior leadership on recruitment, onboarding, performance management, and staff development.
- Ensure integration of clinical, wellness, and health education services within programs for seamless client experience.
- Promote internal referrals and collaborative care across team boundaries.
- Monitor program KPIs and generate reports highlighting improvement opportunities.
- Support adoption of best practices in both virtual and in-person care delivery modalities.
- Oversee compliance with DCHC policies, funder rules, and applicable legislation.
- Maintain community relationships and represent DCHC at external planning tables and collaborations.
- Encourage community involvement in program design and planning activities.
Qualifications and Experience
- Bachelor’s degree in health care, social services, public health, or related discipline; Master’s is advantageous.
- At least five years of experience in community-based health or social services, including leadership or supervisory capacities.
- Proven background in integrated care delivery, primary care, mental health, or related wellness disciplines.
- Strong knowledge of population health, health equity, and social determinants of health.
- Demonstrated ability to lead interprofessional teams in cross-functional environments using matrix management principles.
- Excellent communication, leadership, and organizational capabilities.
- Familiarity with quality improvement methodologies and performance frameworks such as the Quadruple Aim.
- Awareness of Ontario’s health system priorities and ongoing transformation efforts.
- Full COVID-19 vaccination required, with accommodations per human rights legislation.
Additional Information
- Permanent position with 35 hours weekly, including some flexibility for evening and weekend work.
- Primary work locations include Oshawa and Pickering with expectations to rotate across all DCHC sites.
