Centrecare is a not-for-profit organisation employing over 300 staff, dedicated to providing professional counselling, support services, accommodation, family violence assistance, mediation, and justice programs across more than 11 sites in Perth Metro, Goldfields, and Bunbury regions.

Role Overview

The Homelessness Services Connector works with individuals experiencing or at risk of primary homelessness by facilitating connections to suitable accommodation and support services. This role supports the Department of Communities' Find My Way portal, a digital platform providing access to specialist homelessness services in Western Australia. Clients self-refer through this portal for assistance.

Key Responsibilities

• Screen incoming clients and provide appropriate referrals to available services and accommodation providers.
• Engage and coordinate with specialist homelessness service providers to advance client referrals.
• Communicate referral results to service users through an online platform.
• Build and maintain networks with specialist homelessness services in the metropolitan area.
• Keep information about accommodation options and support services current and accessible.
• Accurately input data into the Specialist Homelessness Information Platform.
• Collaborate with clients and services to offer a comprehensive range of referrals.

Candidate Profile

• Possesses a recognized bachelor’s degree in Community Development, Social Work, Social Sciences, or related fields.
• Holds 1 to 3 years of relevant experience in homelessness services and supporting clients affected by family and domestic violence.
• Demonstrated proficiency in computer usage, online software applications, and data collection related to service delivery.
• Skilled in engaging with a diverse client base.
• Possesses a valid Western Australian Driver's license.
• Holds a current National Police Certificate and Working with Children Card.
