About the Role
The Clinical Data Specialist will join a dedicated team responsible for facilitating chart reviews and preparing patient records to support clinical teams. The main objective is to ensure smooth and effective medical care from the patient's initial visit onwards. Collaborating closely with physician chart reviewers and nursing documentation specialists, this role supports seamless access to patient records throughout their healthcare journey with One Medical Senior. This position is integral to the Medicare Population Health Programs team, acting as a centralized point of support for care teams and practice operations.
Key Responsibilities
- Efficiently and accurately retrieve outside medical records by navigating various Electronic Health Records (EHRs), Health Information Exchanges (HIEs), and portals to prepare charts for upcoming patient visits in compliance with standards.
- Identify ways to utilize external data sources, including hospital records, Blue Button, and payer data, to enhance retrospective and prospective documentation accuracy.
- Maintain strong communication and collaborative partnerships with physician and nurse reviewer teams.
- Contribute to identifying operational improvements related to record handling and visit preparation workflows.
- Support departmental productivity and quality goals through general assistance.
- Manage tickets related to medical records and associated processes.
- Perform other duties as assigned to support team objectives.
Required Experience and Skills
- Minimum of one year experience working with Medicare and Medicare Advantage programs.
Preferred Qualifications
- Comfortable working with multiple technical systems; quick to learn and adapt to various software platforms.

Compensation and Benefits
The hourly wage for this position ranges from 20 to 29 USD depending on experience, qualifications, and location. Starting Day 1, employees are eligible for benefits including Employee Assistance Program (EAP), Mental Health Support, Medical Advice Line, and 401(k) matching.
