Job Overview
The Child & Adolescent Case Manager, also known as Community Support Individual (CSI), plays a vital role within the Child & Adolescent Program by delivering recovery-oriented, family-centered, strength-based services tailored for children and adolescents in their communities. This position involves applying treatment interventions aligned with each individual’s recovery plan, managing care coordination, collaborating with various community and agency stakeholders, supporting family education, and promoting utilization of natural supports.
Key Responsibilities
- Conduct comprehensive community and clinical assessments, including Emerging Adult Case Plans, Transition to Adult Care, and Life Skills Assessments for assigned individuals.
- Participate actively in interdisciplinary treatment team meetings across court, community, and educational settings to offer insights grounded in client and family interactions.
- Deliver ongoing support, case management, and skill-building assistance that align with treatment goals, focusing on strengths and preferences.
- Ensure timely and quality documentation of all client interactions, including face-to-face meetings, collateral contacts, and outreach attempts, adhering to agency and regulatory standards.
- Engage regularly with the Program Manager to foster professional development, review service quality, and manage administrative matters.
- Maintain a minimum contact of twice per month outreach with each assigned client.
- Serve as a liaison facilitating communication among community providers to support client needs.
- Communicate effectively with clinicians and agency staff regarding client challenges and concerns.
- Adhere to ethical standards and promptly report any client safety or health issues to the Program Manager.
- Transport program participants to community integration activities using agency vehicles.
- Perform additional duties such as leading group skill services and enhancing community partnerships upon direction from management.
Working Conditions
- Work primarily takes place in community settings.
- Standard schedule: Monday to Thursday from 10:15 AM to 7:00 PM and Fridays from 8:15 AM to 5:00 PM.
- Occasional weekend work required, including availability on the first Saturday monthly from 10:00 AM to 2:00 PM, as well as adjustments for community events.
- Use of office and communication equipment such as computers, phones, fax, copiers, and scanners is required.
- Completion and passing of agency-sponsored defensive driving and other required training courses are mandatory.
Minimum Qualifications
- Bachelor’s degree in fields like counseling, social work, criminal justice, or related disciplines.
- At least two years of experience in behavioral health settings providing case management or community support services.
- Competence with technology including electronic medical records, telehealth platforms, computers, and smartphones.
- Strong care coordination abilities to link individuals with agency and community resources.
- Possession of reliable transportation and strong interpersonal skills, with a demonstrated ability to work collaboratively within a team.
- Pre-employment drug screening is a requirement.
