About the Role
The Case Management Assistant at DCH Health System is responsible for supporting post-acute care planning services for patients in the hospital and emergency department. Working under the guidance of RN or Social Work Case Managers, this role involves coordinating patient transitions and ensuring compliance with healthcare policies.
Key Responsibilities
- Assist RN and Social Work Case Managers with arranging post-acute care services following applicable regulations.
- Coordinate the transfer of complete medical records to post-acute care destinations during business hours.
- Engage with post-acute care facilities to verify bed availability primarily through CarePort and by fax or phone as needed.
- Distribute required regulatory information such as Medicare messages and choice forms to patients and families, maintaining proper filing of documents.
- Immediately inform case managers about patients' selections regarding post-discharge care.
- Organize transportation for patients when necessary and communicate transfer details to families and patients.
- Arrange delivery of durable medical equipment for patients' homes.
- Schedule follow-up physician appointments after discharge.
- Ensure timely and accurate documentation of authorizations, referrals, data entry into CarePort or medical records.
- Serve as a communication bridge between RN and Social Work Case Managers to facilitate information flow.
- Build cooperative working relationships with case management staff, vendors, liaisons, patients, families, and caregivers.
- Conduct follow-up calls after discharge to address patient concerns and refer issues to the appropriate Case Manager.

Requirements
- Minimum of high school diploma or equivalent education.
- Preferred experience using CarePort and Meditech systems.
- Strong clerical and organizational capabilities.
- Excellent verbal and written communication skills, comfortable interacting in person and via telephone.
- Familiarity with medical terminology.
- Proficiency in computer use and data entry, especially Microsoft Word.
- Fluent English reading, writing, speaking, and comprehension skills.
Working Conditions

Physical Demands
- This position requires primarily sedentary work, with occasional lifting or moving up to 10 pounds.

- Physical attendance at the Tuscaloosa, AL location is mandatory.
