Job Overview

This full-time position centers around working alongside a multidisciplinary team to create and implement release strategies for patients transitioning from correctional environments.

Key Responsibilities

• Arrange and coordinate behavioral and mental health services, medical care, and community support resources required post-release.

Qualifications and Requirements

• Bachelor's degree in Psychology, Social Work, Counseling, or a similar discipline.
• Preferred minimum of two years of experience in release planning.
• Background in correctional healthcare, criminal justice, or forensic settings is advantageous.
• Must successfully complete background screenings and secure required security clearance.
• Certification in Basic Life Support (BLS) is mandatory.
• Demonstrated excellent communication skills and effective collaboration abilities.
• Familiarity with community resource networks.
