About the Role
The Canadian Mental Health Association (CMHA) – Toronto seeks a dedicated Case Manager for its East Case Management Program. This program offers intensive case management focused on supporting individuals with mental illness by promoting their independence and quality of life. Through coordination of essential services and ongoing client support, the role emphasizes building compassionate and trusting relationships. Services are delivered within the community setting.
Key Responsibilities
Working under the Program Manager and alongside an interdisciplinary team, the Case Manager will carry out client assessments, formulate and implement individualized care plans, and facilitate access to necessary services. Tasks include intake and needs evaluations, direct service interventions using a strength-based and trauma-informed framework, follow-ups, client advocacy, and fostering partnerships with other community agencies. Regular participation in team meetings and communication with management are also expected.
Required Qualifications and Experience
- Undergraduate degree or diploma in social services or related discipline.
- At least three years of hands-on experience supporting adults with serious psychiatric disabilities, preferably within community mental health.
- In-depth knowledge of crisis intervention, addictions, recovery-oriented practices, culturally competent care, trauma-informed approaches, and severe persistent mental illness.
- Skilled in solution-focused strategies, crisis management, and working inclusively with diverse populations through anti-racism, anti-oppression, and non-homophobic principles.
- Ability to work autonomously and collaboratively with multidisciplinary teams, exhibiting strong assessment, analytical, and decision-making capabilities.
- Valid Ontario Driver’s License with access to a vehicle and appropriate insurance.
Working Conditions and Salary
This full-time position requires a 35-hour workweek. The salary range is $54,943 to $60,203 annually.
