Health Coach

Position Overview

The Health Coach role focuses on assessing and evaluating the wellness needs of members, aiming to support and maintain their optimal health status. This position involves guiding members and their families towards suitable resources that foster their care and wellbeing. The assigned tasks are generally straightforward and possess a moderate level of complexity.

Key Responsibilities

The Health Coach applies diverse strategies, methodologies, and techniques to effectively address member health concerns. This includes identifying and overcoming obstacles that prevent efficient care delivery. Continuous patient monitoring is essential to track progress towards health goals using assessments, data analysis, member interactions, and active care plans. The role requires adherence to professional concepts, standards, regulations, and operational protocols within the designated work area. The Health Coach independently manages work priorities based on established precedents and documented guidelines, with occasional interpretation as necessary.

Qualifications and Requirements

• Possession of a bachelor's degree.
• Minimum of two years' relevant work experience.
• Proficient knowledge of Microsoft Word and Excel.
• Excellent verbal and written communication skills along with strong interpersonal abilities.
• Competency in using various electronic information systems and software applications.
• This position involves direct patient interaction and participation in the Tuberculosis screening program; therefore, candidates must be willing to undergo TB screening.
• Demonstrated enthusiasm for contributing to an organization dedicated to continuous enhancement of consumer experiences.
• Participation in the Driver Safety Program is mandatory. Candidates must hold a valid state driver's license and provide proof of personal vehicle liability insurance with coverage minimums of 100,000/300,000/100,000.

Additional Information

This role requires regular, in-person presence at the Greater Sebring-Avon Park area. Candidates must comply with healthcare and safety protocols related to patient-facing positions.
