Job description
Job Overview
This position is a fixed-term secondment until 31 March 2027 to support service needs. Applicants for this secondment must first secure approval from their current line manager prior to submitting an application. The role is based within the West Community Services Team and involves working as a Clinical Care Co-ordinator covering the Caernarfon, Dolgellau, Amlwch, and Benllech CRT areas.
The Clinical Co-ordinator will facilitate referral triage, coordinate rapid response efforts, and conduct discharge follow-ups. The aim is to ensure patients receive appropriate care promptly, in the right setting, delivered by the right professional. This position requires close collaboration with all team members as part of a multi-disciplinary unit to achieve performance targets and ensure the provision of safe, high-quality care for patients, their families, and staff.
The ideal candidate should demonstrate strong communication ability, be well-organized and proactive, and possess leadership capabilities. Clinical qualifications and skillset requirements are detailed in the job description and person specification.
Main Responsibilities
- Manage a clearly defined caseload, maintaining responsibility over a 24-hour period in accordance with best practices.
- Participate in complex needs assessments including Continuing Healthcare (CHC), Health and Social Care evaluations, and assessment of acutely unwell patients.
- Collaborate effectively with external agencies to deliver patient services autonomously.
- Perform management duties as assigned and provide coverage for the Community Resource Team Leader when required.
Qualifications and Experience
- Must be a Registered Nurse with the Nursing and Midwifery Council (NMC).
- Previous experience working with patients presenting with acute and complex healthcare needs.
- Ability to work collaboratively within the Community Resource Team and with other organizational partners across the health system.
Language Skills
Welsh language skills are desirable but not essential; both Welsh and English speakers are encouraged to apply.
