Overview

This role is designed to enhance patient health outcomes by integrating clinical pharmacy expertise with ongoing quality improvement programs. Key Responsibilities

• Work jointly with healthcare teams to deliver pharmacy care focused on safety, quality, and improved patient outcomes.
• Assist in the rollout of pharmacy guidelines, policies, and quality-focused programs.

• Promote patient medication adherence and mitigate risks linked to high-risk drugs through targeted education and outreach.

• Act as a pharmacy resource for clinical best practices, medication management, and clinical guideline consultation.

• Ensure quality reporting is accurate, transparent, and reliable.
• Develop and deliver presentations and educational programs for healthcare professional development.
• Participate in research projects concerning medication use, quality improvement, and patient safety.
• Support evaluations of medication use and the advancement of pharmacy services.

Qualifications and Experience

• Must hold a current pharmacist license valid in the state where services are provided; remote employees must maintain an active license in their residence state.
• Pharmacist licensure through the appropriate state board is mandatory.
• Residency training or a minimum of three years' pharmacy practice experience is preferred.
• Higher education such as a Master of Public Health or related graduate-level qualification is advantageous.
• In-depth knowledge of pharmacy operations, medication safety standards, and healthcare quality enhancement techniques.
• Ability to interpret clinical guidelines and analyze data to underpin evidence-based decisions.
• Strong communication capabilities, both oral and written.
