Substance Misuse Practitioner

Role Overview

Join our Sexual Health team at Manchester University NHS Foundation Trust, working alongside medical, allied health, nursing personnel, and a dedicated Substance Misuse Practitioner to deliver comprehensive care to individuals affected by ChemSex. You will guide service users from initial contact through their recovery journey, including screening, assessment, recovery planning, and facilitating referrals. Our focus is on reducing harm related to drugs and alcohol both for individuals and the wider community, collaborating extensively with voluntary organizations for ongoing support.

Key Duties

• Manage assigned groups of clients by conducting screening, asset mapping, detailed assessments, and formulating recovery strategies using motivational interviewing techniques.
• Develop and coordinate personalized care packages tailored to each service user.
• Carry out thorough assessments encompassing community resource identification and design clear recovery plans that support substance use reduction, sexual health improvement, and social reintegration.
• Deliver behavior change interventions aimed at enhancing motivation, confidence, and personal strengths.
• Ensure availability and maintenance of intervention resources.
• Work collaboratively with other healthcare professionals and external services to address clients' broader health, mental health, and sexual assault referral needs.
• Accurately record, collect, and report relevant data in a timely manner and participate in multidisciplinary team discussions.
• Conduct risk assessments and manage associated risks.
• Perform triage assessments with onward referrals to appropriate treatment and community support agencies.
