Responsibilities

  • Coordinate activities that help individuals plan for, discover, secure, and sustain the supports they want.
  • Advocate for individuals and support them in self-advocacy.
  • Encourage meaningful participation in team meetings and support individuals who choose to lead those meetings.
  • Help negotiate disagreements and resolve conflicts within the team.
  • Develop, review, and monitor Individual Plans in line with COMAR requirements and agency policy.
  • Visit assigned individuals after annual planning meetings at least four times during the IP year, or more often when the level of support needed requires it.
  • Assist with transfers and transitions quickly enough to maintain continuity of services and supports.
  • Address provider incident reports promptly so health and safety concerns are handled appropriately.
  • Record consumer-related activity according to agency procedures to support accurate billing.
  • Ensure services are documented thoroughly enough to meet agency billing expectations.
  • Identify and pursue alternate supports and funding sources to meet individual needs.
  • Attend and participate actively in meetings assigned by the Team Leader.
  • Complete required training under agency policy and DDA regulations and continue building professional skills through internal and external learning opportunities.
  • Submit required administrative materials such as timesheets, schedules, and reports on time.
  • Follow all applicable state and federal rules related to targeted case management.
  • Carry out additional related duties as assigned to support effective agency operations.
