About the Role
Join a healthcare organization focused on helping patients receive compassionate care in the comfort of their homes. The company is a large independent Medicare-certified home health provider in Florida with more than 20 years of experience delivering patient-centered care. This position supports documentation management and helps keep referral and order workflows accurate, complete, and on time.
Key Responsibilities
- Visit referral partner offices alongside CCs to build working relationships, understand how each partner sends referrals, and identify the delivery method they prefer. Keep delivery-method records updated as needed.
- Make routine field visits to referral partner offices to secure signatures on current or pending orders.
- Coordinate with referral partners to collect missing paperwork when a CC is no longer assigned to the agency or branch.
- Check that physician details are accurate on profile pages and investigate and fix any errors found.
- Submit Smartsheet updates for changes to CC referral sources such as physicians, facilities, hospitals, or any other information that needs revision.
- Record daily work completed to obtain current or pending orders, including the date, action taken, and follow-up date, and review the prior week’s activity.
- Support order escalation efforts by coordinating with the home office, referral partner, and CC.
- Join meetings to provide status updates on outstanding face-to-face items, hand-carry items, orders, and escalations, and share plans for obtaining missing documentation.
- Print and organize orders for each CC and review a weekly plan for obtaining signatures. Be ready to send outstanding orders, face-to-face items, or hand-carry orders to a CC when they are visiting a referral source and can obtain a signature during that visit.
- Run weekly face-to-face and order reports and partner with the CC to make sure orders are completed within 14 days or within the timing appropriate for the patient’s case.
- Review accounts that may require write-off with RDS and determine the next step for resolution or write-off submission.
- Communicate professionally across departments to resolve unfinished orders.
- Respond quickly to questions, concerns, and discrepancies with timely resolutions.
- Follow HIPAA requirements for all accounts and documentation handled.
Requirements
- High school diploma or equivalent is required.
- Prior experience using an EMR system is required.
- Strong communication, organization, and time-management abilities.
- Advanced typing skills, solid computer proficiency with MS Office, and the ability to manage several software tools at once while protecting data accuracy.
- Comfortable working in a deadline-driven environment.
- Able to stay productive under heavy workload pressure and tight timelines.
