Opportunity Overview
The Associate Discharge Planner role is designed to support the care management team by coordinating patient discharge planning and assisting with administrative responsibilities to ensure seamless transitions from hospital stays to home or other care settings, thereby contributing to continuous and high-quality patient care.
Key Responsibilities
- Organize discharge planning activities to ensure smooth patient transitions.
- Provide administrative support for care management department operations.
- Work collaboratively with multidisciplinary teams to tailor discharge plans according to patient requirements.
- Assist in maintaining accurate documentation and clear communication throughout discharge processes.
Qualifications and Requirements
- Experience in clinical or administrative roles within healthcare settings.
- Associate's Degree preferred but not mandatory.
