About the Role
CareHarmony is a venture-funded startup focused on helping physicians succeed in the value-based care landscape. The company combines modern population health technology with around-the-clock care coordination services to support better outcomes for chronically ill patients. Its mission is to improve healthcare and set a high standard for chronic care management.
The Enrollment Specialist sits on the Patient Engagement Specialist (PES) team. In this role, you will contact patients by phone, explain a Medicare-funded care coordination program, and guide eligible patients toward enrollment so they can better manage chronic conditions such as diabetes, hypertension, COPD, and similar illnesses.
Key Responsibilities
You will work from a designated patient list and conduct outbound calls to educate patients about the program and encourage participation.
A major part of the job is building trust quickly, adapting your message to each patient’s age, health concerns, and insurance situation, and presenting the program in a clear, persuasive, and compassionate way.
Schedule and Work Setup
This is a fully remote position, allowing you to work from home. The schedule is full-time, Monday through Friday.

Requirements
Candidates should bring strong interpersonal skills, the ability to handle a fast-paced high-volume sales environment, and a track record of meeting deadlines and performance targets.
You should be able to spend more than 80% of your workday on the phone, communicate in a warm and professional manner, and explain the program in a concise, informative, and engaging way.
Additional expectations include teamwork, leadership potential, accurate data entry, multitasking ability, empathy, and comfort using common office tools such as Outlook, Excel, Word, and Skype.
Applicants must be available during business hours from 8:00 AM to 5:30 PM CST and must be based in the United States.
An associate degree or above is required.
Physical Requirements
This job is largely sedentary and involves sitting for extended periods.
You must be able to speak clearly, listen carefully, and take detailed notes in real time during long telephone conversations.
