About the Role

The Quality Improvement Specialist serves as a bridge between patients, provider practices, and internal teams to spot quality gaps, coordinate follow-up, and help close those gaps through education, outreach, and data-informed collaboration. This role manages a defined patient and provider panel, shares performance insights, and supports improvement on HEDIS and Medicare Stars measures using payer-specific rules, timelines, and attribution logic. It is well suited to someone who is comfortable in a fast-moving, performance-driven value-based care setting.

What You Will Do

  • Act as a reliable partner to nephrology, primary care, and specialty practices on HEDIS and Stars expectations, documentation practices, and Evergreen’s value-based care priorities.
  • Review and explain quality reports, care gap lists, and dashboards, turning performance data into practical next steps that fit each practice’s workflow and capacity.
  • Train provider practices on accurate documentation, complete coding, and effective workflows that influence HEDIS and Stars results, including timing requirements and acceptable data sources.
  • Lead recurring meetings with providers and practice staff to examine trends, remove workflow blockers, and create action plans for improvement.
  • Help practices understand and adopt Pay-for-Performance elements tied to HEDIS and Stars by clarifying incentives, expectations, and measure details.
  • Spot risks or gaps that may affect outcomes, escalate issues when needed, and work with internal teams to support timely intervention.
  • Track and manage an assigned patient panel, focusing on gaps in preventive care, chronic disease management, and care coordination.
  • Reach out proactively to patients by phone, electronic channels, and other approved methods to educate, motivate, and support completion of screenings, labs, immunizations, specialty visits, and other recommended care.
  • Help coordinate patient care by serving as a key point in closing quality gaps and improving overall patient wellbeing.
  • Identify barriers at the patient level, such as transportation challenges, scheduling issues, unclear care plans, limited health literacy, or other social barriers, and use internal resources or escalation pathways when appropriate.
  • Reinforce provider-directed care plans and maintain timely, clear communication among patients, practices, and Evergreen care teams.
  • Measure outreach results, monitor gap closure progress, and document patient interactions accurately and on time.
  • Work closely with Quality, Clinical Operations, Coding & Documentation, Practice Transformation, and Pharmacy teams to deliver payer-aligned support.
  • Take part in cross-functional quality initiatives aimed at improving performance at scale.
  • Maintain accurate and timely records in EMRs and quality systems.
  • Follow Evergreen compliance standards, HIPAA rules, and all applicable regulatory requirements.
  • Complete other duties that support Evergreen’s value-based care and quality improvement goals.
  • Perform additional tasks as assigned that are consistent with the responsibilities of this position.

Required Qualifications

  • 1 to 2 years of experience in quality improvement, population health, provider engagement, or care coordination.
  • Experience interacting directly with patients through outreach.
  • Working knowledge of quality workflows, care gap closure processes, and documentation standards.
  • Strong communication skills and the ability to build productive working relationships.
  • Comfort using EMR systems and healthcare technology tools.
  • Intermediate proficiency with Microsoft Office products, including Outlook and Teams.
  • Ability to work effectively in a mostly remote setup.
  • Home internet must provide at least 25 Mbps download and 10 Mbps upload; hardwired Cable, Fiber, or DSL connections are recommended.
  • Evergreen will supply remote employees with telephony applications and equipment needed for the role.
  • A dedicated workspace with no ongoing interruptions is required to protect member PHI/HIPAA information.

Preferred Qualifications

  • Practical experience with HEDIS and Medicare Stars measures, including identifying and closing care gaps.
  • Background in nephrology, CKD/ESRD, or chronic disease management.
  • Medical Assistant or LPN license.
  • Bachelor’s degree in a health-related field is preferred, or equivalent healthcare experience.

Compensation

The pay range for this position is $20 to $24 per hour. Actual compensation will depend on experience, education, demand for the role, and other position-specific factors. This role is also eligible for an annual bonus.
