Overview
Join Fuze Health’s Specialty Operations team as a Patient Care Pharmacist focused on specialty pharmacy services. We prioritize patients and strive to enhance healthcare quality by integrating advanced digital health, diagnostics, and pharmacy technologies. This role involves autonomous dispensing activities, patient consultations, clinical assessments, and process enhancements in a collaborative, multi-state team environment.
Key Responsibilities
- Execute patient management programs adhering to URAC, ACHC, VIPPS standards using various communication methods including phone, text, app messaging, and email.
- Conduct initial patient assessments by reviewing medical chart notes to evaluate prescribing appropriateness.
- Monitor patient adherence, treatment efficacy, adverse reactions, and drug interactions through clinical check-ins.
- Identify candidates who require clinical interventions and collaborate with healthcare providers to optimize therapeutic outcomes.
- Manage prescription transfers to suitable pharmacies when unable to fulfill orders, ensuring smooth care transitions.
- Maintain communication of prescription statuses to both patients and providers.
- Apply comprehensive knowledge of pharmacy billing for Medicaid, Medicare, ADAP, commercial insurance, and assistance programs to address patient cost challenges.
- Detect and communicate any concerns regarding scheduling and delivery procedures proactively.
- Create and distribute drug information resources to improve Specialty Operations performance.
- Stay current on regulatory and industry standards, initiate appropriate responses, and keep relevant personnel informed.
- Collaborate effectively with local and distributed teams to meet organizational compliance and business objectives.
Qualifications
- Minimum five years’ pharmacist experience, or at least three years within a specialty pharmacy setting.
- Licensed and in good standing as a Pharmacist in Colorado or Texas.
- Strong proficiency with software including MAC OS, Google Apps, and databases for process improvement.
- Excellent interpersonal and communication skills with an enthusiasm for delivering outstanding patient service via multiple communication channels.
- Ability to work independently, demonstrate initiative, and follow through without close supervision.
- Thorough knowledge of relevant state and federal pharmacy laws.
- Capability to evaluate prescriptions considering patient age and disease-specific factors.
- Strong adaptability to implement and refine new protocols efficiently.
Preferred Experience
- Certification as a Specialty Pharmacist such as AAHIVP or CSP.
- Specialized training or familiarity with specialty drugs and disease states like Biologics, LDDs, HIV/PrEP, MS, and Oncology.
- Pharmacy manager or Pharmacist in Charge experience.
- Licensure in multiple states.
Additional Details
- Annual salary range between $114,000 and $171,000.
- No commission or equity participation applicable.
- This role requires residency in specified U.S. states including Arizona, Arkansas, California, Colorado, Florida, and others.
- Employment eligibility requires authorization to work in the U.S.; visa sponsorship is not available.
- The position involves no travel.
