Key Responsibilities

  • Conduct initial client evaluations to establish individualized care plans focusing on the client's specific needs.
  • Periodically review and revise care plans to reflect any changes in client conditions.
  • Carry out supervisory visits at clients' residences to monitor caregiver performance and ensure adherence to the care plan.
  • Offer guidance, continuous support, and in-service training sessions to caregivers to maintain high-quality service delivery.
  • Prepare thorough documentation of assessments, updates, and supervision visits in compliance with agency and regulatory standards.
  • Collaborate closely with clients, their families, and agency personnel to coordinate care effectively and make necessary adjustments.
  • Ensure compliance with relevant regulations and professional standards of nursing practice.
  • Engage in quality assurance processes and assist with audits or regulatory surveys as required.

Qualifications and Requirements

  • Must hold a current and valid Registered Nurse license in North Carolina.
  • Possess at least 12 to 24 months of nursing experience, preferably in home care or geriatric care settings.
  • Strong skills in client assessment and accurate documentation.
  • Excellent communication abilities and interpersonal skills.
  • Capability to supervise and provide support to non-clinical personnel.
  • Detail-focused with effective organizational and time management skills.
  • Possess a valid driver’s license and reliable transportation to conduct home visits.
  • Knowledgeable about state home care regulations and best practices.
