Nurse Educator (LPN) - Hybrid Position

Role Overview

We are seeking a licensed practical nurse to join our team as a Nurse Educator in a hybrid capacity, combining remote and onsite work to enhance patient engagement focused on managing chronic conditions. The core mission is to educate patients on disease management and medication adherence while linking them to pertinent resources.

Key Responsibilities

• Deliver patient-focused education about disease states and medication regimes.
• Facilitate effective communication between patients and healthcare providers to support care coordination.
• Encourage treatment adherence and development of treatment routines.
• Utilize virtual communication tools including calls, emails, and video conferencing to maintain patient engagement.
• Address barriers related to access, financial issues, and reinforce behavioral changes to support long-term therapy adherence.
• Coordinate with various resources to optimize patient care and provide ongoing training and support.
• Assist patients in decision-making regarding treatments, aiming to improve their overall quality of life.

Qualifications

• Active Licensed Practical Nurse (LPN) license in North Carolina.
• Minimum of 3 years clinical support experience in healthcare environments preferred.
• Sound knowledge of pharmacology.
• Proficiency in Microsoft Office suite.
• Strong skills in time management, presentation, and interpersonal communication.
• Hold a valid driver's license and be able to travel as necessary, including flexibility for varying shifts and across multiple time zones.
• Experience in virtual patient education and bilingual abilities are desirable but not mandatory.
