Job description
Role Overview
The interRAI Care Needs Facilitator position serves as the primary advocate and coordinator for interRAI procedures within a Community Healthcare Network (CHN) or Integrated Care Programme for Older People (ICPOP) team. This individual will act as the main point of contact for interRAI activities and maintain communication with the Community Healthcare Organisation (CHO) Educator.
Key Responsibilities
- Serve as the interRAI champion at the CHN or ICPOP team level, facilitating the integration and use of interRAI assessments.
- Conduct care needs assessments and develop care plans to support team functions.
- Help free up team members to attend training sessions by taking on assessment duties as required.
