Job description

Role overview

This position is centered on delivering everyday resident support and personal care in line with each person’s assessment, care plan, and supervisor guidance. The role focuses on helping residents with comfort, safety, hygiene, mobility, communication, and routine life tasks while maintaining a respectful and calm care environment.

Key duties

Responsibilities include supporting residents with daily living activities, documenting care accurately, observing and reporting changes in condition, and coordinating closely with nursing and leadership teams. The role also involves helping residents settle into rooms, assisting with transfers and transport, keeping rooms and personal items organized, and responding promptly to call lights and care needs.

  • Use PCC to confirm resident details when providing assistance with ALDS, meals, and related care tasks.
  • Escalate changes in a resident’s condition to the Health and Wellness Director without delay whenever possible.
  • Complete notes, SPAs, and incident documentation in a clear and detailed way.
  • Report any accident or incident observed during the shift to the Executive Director and Health and Wellness Director.
  • Protect resident health information and immediately report any suspected privacy breach.
  • Raise any known or suspected unauthorized attempt to access the community’s information system.
  • Carry out only those care activities for which you have been trained.
  • Prepare resident rooms, including making the bed before arrival.
  • Welcome residents, escort them to their rooms, and make introductions as needed.
  • Help residents feel comfortable and settled.
  • Inventory and label personal belongings as directed, and store clothing appropriately.
  • Support packing of belongings during room changes or discharge.
  • Move residents to new rooms or receiving areas as needed.
  • Help with loading and unloading residents to and from vehicles when required.
  • Maintain a warm, caring, calm, and positive unit atmosphere.
  • Attend regular shift meetings with nursing staff to address issues and improve service.
  • Communicate resident complaints and grievances.
  • Participate in receiving and sharing the nursing report.
  • Follow infection-control and blood/body fluid exposure procedures.
  • Make beds and add extra covers when requested.
  • Check residents who cannot call for assistance on a frequent basis.
  • Respond quickly to resident calls for help.
  • Routinely assess whether residents’ personal care needs are being met.
  • Help residents understand meal arrangements, including describing tray contents and temperature when vision is limited.
  • Keep water pitchers clean, filled, and within easy reach on every shift.
  • Provide post-meal care such as clearing trays and cleaning hands, face, and clothing.
  • Follow all assigned tasks according to policy, procedure, and supervisor instructions.
  • Work in line with assigned schedules and job assignments.
  • Coordinate with other departments and facility personnel to maintain resident services.
  • Notify the facility if you will be late or absent.
  • Report occupational exposure to blood, body fluids, infectious materials, and hazardous chemicals as required by policy.
  • Support personal care needs such as oral care, bathing, grooming, dressing, and undressing.
  • Keep residents clean and dry by changing soiled clothing, gowns, and linens promptly.
  • Assist with preparation for lab work, x-rays, therapy, dental visits, and other medical tests.
  • Help lift, turn, move, position, and transport residents using beds, chairs, tubs, wheelchairs, and lifts.
  • Carry out restorative and rehabilitative tasks as instructed.
  • Prepare residents for physical examinations.
  • Observe residents to ensure their care needs are being met in line with their wishes.
  • Watch for pressure areas and skin breakdown and report concerns early to help prevent pressure injuries.
  • Report wounds, unexplained injuries, and skin tears.
  • Observe disoriented or comatose residents and document/report findings as directed.
  • Perform special treatments when instructed.
  • Immediately notify the Nurse if a resident leaves or is missing from the facility.
  • Use only equipment you are trained to operate and report defects to the Nurse Supervisor or Charge Nurse.
  • Safeguard all resident care information and report any suspected unauthorized disclosure.
  • Report all allegations of abuse or misappropriation of resident property.
  • Respect a resident’s refusal of treatment and pass the request to your supervisor.
  • Provide back rubs, grooming support, nail care, shaving, and other comfort-related assistance as directed.
  • Help with bowel and bladder care, including bathroom support, bedpans, urinals, and commodes.
  • Track intake and output when required and keep incontinent residents clean and dry.
  • Observe and report bowel movements and stool characteristics as instructed.
  • Use protective and adaptive equipment as directed.
  • Support ear and eye care, including hearing aid insertion and cleaning of glasses or hearing aids.
  • Weigh and measure residents and record temperatures, pulse, and respirations as instructed.
  • Assist residents at end of life and provide post-mortem care when needed.

Supervision

This role does not include supervisory duties.

Qualifications

A minimum of a high school education, or an equivalent qualification, is required. The position also calls for the ability to read and interpret business, technical, and regulatory material; write reports and correspondence; communicate effectively with managers and employees; apply basic math concepts; solve practical problems; and follow instructions presented in multiple formats. Proficiency with Microsoft Suite products is needed.

Physical and workplace requirements

The job may require occasional lifting or moving of up to 25 pounds. It involves prolonged computer use and regular sitting, standing, walking, speaking, and listening. Frequent use of office equipment such as a copier/scanner/fax, telephone, and calculator is expected. The environment is generally quiet to moderately noisy.
