Role summary

The Care Coordinator supports care management programs by conducting field outreach and helping enrollees make healthier lifestyle choices, with the goal of reducing both short-term and long-term effects of chronic conditions.

Core responsibilities

  • Handle both incoming and outgoing calls as part of the role.
  • Deliver comprehensive services to enrollees in a responsible and consistent manner.
  • Complete general assessments for review by a supervisor or lead, who will interpret the results and assign cases to a case manager or health coach as appropriate.
  • Take part in interdisciplinary case reviews to support collaborative assessment and care planning.
  • Contribute proactively to quality management program activities owned by the Health Services team and help maintain program accreditations.
  • Support the Lead, Supervisor, and/or Manager in meeting contractual financial obligations by ensuring services are delivered efficiently and cost-effectively, and by submitting accurate and timely work hours and expenses.
  • Follow all corporate policies, including HIPAA Privacy and Security Rules.
  • Work a schedule between 7:00 AM and 7:00 PM Pacific Time, Monday through Friday; the current hiring need is for coverage from 10:00 AM to 7:00 PM.
  • Work remotely from within Oregon and be available for up to 5%–10% travel to the Tualatin office if business needs require it.

Requirements

  • Associate’s degree in a health-related field or medical assistant training.
  • At least 4 years of care coordination experience, preferably in a field setting.
  • At least 2 years of experience in a clinical environment, ideally involving field-based delivery.
  • Preferred: 1 or more years of telephonic outreach or call center experience.
  • Preferred: Spanish bilingual ability.
  • Preferred: experience in public sector programs such as Commercial, Medicare, or Medicaid.
  • Working knowledge of care management principles, ethics, and service delivery practices.
  • Understanding of customer service standards.
  • Familiarity with medical terminology.
  • Awareness of regulatory and accreditation requirements.
  • Strong written and verbal communication abilities.
  • Strong interviewing skills for both field and phone-based interactions.
  • Good computer proficiency, including care management systems, web tools, and Microsoft Office applications such as Word, Excel, and Access.
  • Strong organizational and prioritization skills.
  • Ability to use purposeful, outcomes-driven critical thinking in day-to-day work.
  • Ability to receive feedback professionally and apply changes to improve performance and productivity.
  • Ability to interpret current and historical information to guide work responsibilities.

Additional information

This position is fully remote within the state of Oregon. Depending on business needs, travel of up to 5%–10% to the Tualatin office may be required.

Pay depends on where the candidate falls within the range, which is influenced by relevant experience and skill level.
