These offerings include schools, training and employment development centers, home-based care, and community services addressing the physical, psychological, environmental, and social needs of PWIDs.
Job Role Overview
The Transition Support Officer (TSO), located at various MINDS Regional or Community Hubs, delivers practical, hands-on support and training to clients experiencing transitions such as moving from school to community hubs, or between community hubs and residential care settings. The TSO works closely with the Hub Manager and multidisciplinary teams including Allied Health Professionals, Training Officers, Vocational Coaches, and Learning Support Trainers to develop and revise Individualized Development Plans (IDP). Collaboration with Social Workers, Care Coordinators, and Trainers ensures caregivers are regularly engaged to facilitate skill transfer into home or community environments and keep caregivers updated on client progress.
Primary Responsibilities
- Assist the Hub Manager in creating, reviewing, and updating client IDPs using a person-centred method.
- Participate in transition planning meetings, case reviews, and IDP meetings together with AHPs and other involved parties.
- Ensure clients achieve their IDP goals aligned with their long-term interests and aspirations.
- Engage with clients and caregivers regarding transition discussions and training needs in partnership with Social Workers.
- Coordinate family meetings to review short- and long-term goals.
- Provide hands-on training tailored to client transition requirements in appropriate settings, meticulously documenting training progress and modifications.
- Collaborate with multidisciplinary teams, caregivers, and community partners including volunteers to advocate for client needs encompassing environmental, social-emotional, and behavioral support.
- Maintain detailed records of engagements and interventions across stakeholders.
- Build and consolidate a resource bank of person-centred practices and tools to facilitate collaborative professional development.
- Evaluate and reassess client IDPs to support sustainable development across their lifespan.
- Conduct client assessments to measure transition and development progress using pre- and post-intervention tools.
- Adapt IDP goals and plans in cooperation with teams if outcomes fall short or to elevate client independence.
- Assist the Transition Coordinator by documenting client outcomes and generating summary reports and dashboards on IDP achievements.
Requirements
- Diploma minimum in Social Work, Psychology, Community Services, or related disciplines.
- At least two years of relevant professional experience.
