Role Overview
Situated in Novena within Ren Ci Hospital, this role focuses on streamlining patient admission processes to optimize bed utilization across all hospital facilities.
Key Responsibilities
- Assess incoming referrals by reviewing medical and nursing details to determine patient eligibility promptly.
- Complete referral evaluations within one working day and collaborate with medical teams for complex cases as needed.
- Coordinate with Medical Social Workers and Nurses for referrals from nursing homes and chronic sick units, obtaining clarifications from referring institutions when necessary.
- Communicate outcomes of referral assessments and suggest alternative options to referring entities if appropriate.
- Expedite admission activities by assigning beds, informing referring institutions about admission specifics, and confirming patient transfer to wards.
- Liaise closely with Nursing Home administrative staff, Business Office, and Nursing teams to support smooth admission processes.
- Ensure all relevant clinical data accompanies the patient upon admission and provide detailed guidance on hospital services to patients and caregivers.
- Monitor upcoming discharges to proactively manage bed occupancy rates, aligning them with budgeted targets for financial planning.
- Accurately collect daily data supporting performance evaluations and generate routine reports and presentations.
- Prepare and submit reports timely to the Ministry of Health (MOH) and update department leadership, management, and regulatory bodies as required.
- Lead or contribute to hospital-wide quality improvement initiatives and recommend process enhancements to boost operational efficiency.
- Undertake additional duties as assigned by supervisors.
Qualifications and Skills Required
- A diploma or degree in healthcare or social service disciplines is preferred.
- Minimum of three years working experience within healthcare or social care sectors.
- Strong interpersonal and communication capabilities.
- Ability to work both independently and collaboratively within a team environment.
- Outstanding organizational skills with meticulous attention to detail and effective time management.
