Role Summary

As a Patient Navigator, you will collaborate with the Care Team to assist patients on their family-building journey. Working closely with nursing staff, you will provide patients with clear guidance and information to navigate their treatment cycles confidently. Your role simplifies complex and sometimes confusing procedures, ensuring smooth handoffs to nurses for cycle management.

Key Responsibilities

• Create patient journey maps and calendars based on individualized treatment plans.
• Manage, monitor, and document completion of tests and diagnostic procedures.
• Establish trust and positive relationships with new patients.
• Translate complex medical terminology and procedures into understandable information.
• Track patient compliance and ensure readiness for treatment.
• Coordinate and schedule patient appointments efficiently.

Qualifications

• Bachelor's degree is required.
• A strong passion for reproductive healthcare.
• Excellent communication skills, both oral and written.

Employment Details

This is a full-time, non-exempt position based onsite in Sunnyvale, CA. The schedule includes early hours, rotational weekends, and holidays.

Compensation

The hourly pay range starts from $25 to $29, determined by factors such as relevant knowledge, skillset, experience, certifications, and educational background.
