The Patient Access & Intake Coordinator is essential in ensuring a streamlined patient admission process. This position supports the Business Development team and the Central Access and Authorizations Team (CAAT) by managing referral activities efficiently and initiating referrals promptly.
Key Responsibilities
- Act as a critical member of the CAAT, exclusively managing patient referral intake and laying the groundwork for admissions.
- Launch patient referrals by creating patient accounts within the relevant referral management platforms.
- Enter demographic and admission details accurately into referral systems like Meditech.
- Collaborate with the Business Development team to collect comprehensive patient and facility information from multiple referral sources.
- Validate and input complete data to facilitate benefit verification, prior authorizations, and patient admissions.
- Manage care transition program referrals in partnership with business development.
- Coordinate with facilities on bed assignments and admission paperwork.
- Confirm completion of insurance verification and authorizations.
- Ensure patient profiles and PIC days are accurately maintained in systems of record.
- Communicate patient-specific needs regarding equipment, beds, pharmacy, and consult services.
- Keep hospital teams updated on admission statuses.
- Coordinate bed availability with facility house supervisors.
- Maintain and reconcile the pending referral log accurately within referral platforms.
- Engage in ongoing professional development and continue improving CAAT admission processes.
- Master scheduling and pre-registration workflows in Meditech and other platforms used by CAAT.
- Perform additional related duties as assigned within team scope.
Required Qualifications and Skills
- High school diploma or GED is mandatory; associate or bachelor's degree is preferred, ideally in a healthcare-related field or with equivalent healthcare experience.
- Prior experience in healthcare or preferably post-acute care is advantageous.
- Excellent communication skills, both verbal and written, with a professional demeanor.
- Strong computer proficiency, including data entry accuracy and familiarity with proprietary software.
- Ability to function as a reliable, professional team member and adhere strictly to company policies, compliance agreements, and federal/state regulations.
- Fluent English reading, writing, and speaking skills.
- Regular attendance and punctuality are essential.
