About the Role
This position focuses on reviewing, organizing, and analyzing inpatient and outpatient medical records to make sure patient documentation is complete, accurate, secure, and aligned with healthcare compliance standards. The role is important for preserving the quality and reliability of health information and supporting safe care delivery.
Key Responsibilities
- Sort and compile discharged patient charts according to the department’s documented process.
- Review inpatient and outpatient files for completeness and adherence to documentation rules.
- Spot missing or incorrect chart entries and coordinate corrections with doctors and nursing teams.
- Track incomplete medical records and keep the follow-up system updated.
- Chase pending documentation from physicians until outstanding items are resolved.
- Handle filing, scanning, retrieval, and long-term archiving of medical records and related reports.
- Ensure requested records are located, delivered, and returned without delay.
- Prepare monthly summaries covering documentation gaps and compliance status.
- Carry out quality checks on records that are still awaiting final completion.
- Look into misplaced, missing, or incorrectly stored records and resolve filing issues.
- Confirm patient details and maintain accurate indexing and filing practices.
- Protect the confidentiality and security of all patient health information at all times.
- Answer questions related to medical records and internal procedures.
- Assist with day-to-day department operations and take on other assigned tasks as needed.
Requirements
- A relevant bachelor’s degree in Health Information Management, Nursing, or another healthcare-related discipline.
- Prior medical background is mandatory.
- Solid understanding of medical terminology and healthcare documentation practices.
- Strong English communication skills in speaking, reading, and writing.
- Excellent accuracy, organization, and commitment to confidentiality.
- Comfort working with medical records systems and electronic documentation tools.
- RHIA certification is preferred.
- RHIT certification is preferred.
- CCS or CCA coding certification is required.
- At least 5 years of experience in a Medical Records Department.
- Experience in record analysis, chart assembly, or health information management is an added advantage.
- Familiarity with JCI standards and healthcare documentation expectations will be beneficial.
