About the Role
Located in Brooklyn, NY, this opportunity is with a rapidly expanding home care agency seeking a seasoned Director of Coordination. The role demands leadership over all care coordination functions to guarantee continuous and efficient patient service, effective team management, streamlined operations, and adherence to New York State's home care regulations.
Key Responsibilities
- Direct and oversee Care Coordinators and Coordination Supervisors, fostering accountability and achieving high team performance.
- Ensure daily staffing fully addresses all authorized patient cases without gaps.
- Manage staffing challenges including callouts, emergencies, overtime, and cases that are difficult to assign.
- Supervise live-in caregivers and split cases while maintaining compliance with labor laws and company protocols.
- Collaborate closely with HR, Payroll, Recruitment, Nursing, Intake, Authorizations, and Retention departments to resolve staffing issues.
- Maintain detailed and accurate records of scheduling modifications, coordination procedures, and case documentation.
- Address and resolve scheduling concerns raised by caregivers and patients effectively.
- Track and analyze critical performance indicators such as staffing coverage, overtime rates, response times, and complaint volumes, then formulate strategies for continuous improvement.
Candidate Requirements
- Minimum of five years’ leadership or management experience within a home care agency setting.
- At least two years’ direct experience in home care coordination.
- Comprehensive knowledge of New York State-specific home care regulations and labor law compliance.
- Demonstrated ability to manage large coordination teams under high-pressure, fast-paced conditions.
- Proficiency in using HHA Exchange software.
- Superior leadership capabilities combined with strong communication and problem-solving skills.
- Experience managing coordination teams across multiple regions or with high case volumes is strongly preferred.
Compensation and Location
This full-time, onsite position is based in Brooklyn, NY, offering an annual salary between $120,000 and $140,000.
