Overview
Copley Hospital, located in Morrisville/Stowe, Vermont, seeks a Community Health Integration Manager to join their Quality Department team. This role is responsible for supporting the creation and execution of organizational strategies aimed at achieving population health objectives through leadership of the Blueprint and Community Health Team initiatives. Position availability includes full-time, part-time, and per diem shifts, with part-time considered here.
Responsibilities
- Lead the Community Health Team to execute population health strategies via integrated workflows.
- Establish structures to monitor team objectives, including team huddles and workplans.
- Conduct manager meetings, individual check-ins, and performance reviews with team members.
- Design and implement strategies addressing Community Health Needs Assessment findings and collaborate with community stakeholders.
- Manage community partnerships to enhance cooperation between primary care providers and community organizations.
- Advocate for community collaborative initiatives and promote awareness of Community Health Team and population health programs externally.
- Develop and maintain referral protocols with community partners.
- Oversee Blueprint projects alongside the Blueprint Facilitator and attend relevant meetings.
- Support programs such as Pregnancy Intention Initiative, Self-Management Programs, Community Health Team Support, and MOUD program.
- Administer Community Health Team budget including grant and Blueprint funding; track expenses such as training and travel.
- Collaborate on strategies aligned with clinical and quality initiatives, leveraging ACO and payor partnerships to enhance care coordination and health outcomes.
- Utilize care coordination to meet quality measure goals for targeted patient cohorts.
- Support initiatives to manage Emergency Department usage, inpatient admissions, and hospital readmissions.
- Lead new projects and develop implementation strategies for Community Health Team and Blueprint programs as needed.
Qualifications
- Expertise in project management, team leadership, strategic planning, meeting facilitation, and budget administration.
- Exceptional communication and collaboration abilities.
- Strong critical thinking and problem-solving skills.
- Registered Nurse credential preferred.
- Bachelor’s degree in a related discipline required.
- Minimum of five years’ experience managing projects, teams, and strategic development, with proven work alongside community partners and leadership in collaborative efforts.
